Mrs. Medina is Randall’s health clerk. If your student has any medicine they need to take during school hours, please reach out to her. In order for Mrs. Madison to properly and safely care for your student, we must have your child’s care plan on file, along with the appropriate medication authorization form.
Please review the three forms below:

• Self-Carry/Administration Form – For students who are able to safely carry and administer their own medication at school.
• Medication Administration Form – For students who require an adult on-site to assist with or administer their medication.
• Anaphylaxis/Allergy Form – For students with allergies that may result in an anaphylactic reaction.

Please complete one form for EACH medication your child requires at school.
After selecting the appropriate form, please forward it to your child’s physician for completion. Once the physician has completed the form, you may return it to the school through ParentSquare, email, or in person at the school office.
